Responsibilities
Conduct assessments to identify clients’ needs and develop tailored care plans. Provide counselling and emotional support to individuals facing challenges. Liaise with external agencies and professionals to coordinate services for clients. Monitor and evaluate clients’ progress towards goals. Advocate for clients’ rights and access to services. Maintain accurate and confidential client records
Qualifications
A recognised Social Work qualification. Registration with the Health and Care Professions Council (HCPC). Experience in mentoring or supervising colleagues is desirable. Strong communication and interpersonal skills. Ability to work effectively in a multidisciplinary team. Empathy, compassion, and a non-judgemental approach
Contract length: 3 months
Pay: £37,246.00-£39,106.00 per year
Expected hours: 18 – 36 per week

Schedule: Monday to Friday, Overtime, Weekend availability.
Licence/Certification: Driving Licence (preferred)
